Consent/Data Information Online Form
We are required to annually review and update the consent/data information we hold on record for our students. This includes consent for medical emergency permission, photos, sports events and trips, without which your child would not be able to participate in any offsite events.
This year, in order to streamline the system and move towards our promise for a greener environment, we have introduced an online system. The revised form will mean that you will no longer need to give consent to each individual trip/visit unless it is a residential, for which an additional form will be required.
To complete and return your child’s details please click on the following link to access the form https://help.clystvale.org/?page_id=12877. Please note that you will need to complete a form for each child you have at the College.
We will email you a request for this to be completed by the end of June annually.

Annual Offsite Visits Consent/Information Form
In order to reduce the burden of paperwork for parents/carers, this will be the only consent form issued for all trips and visits. When an event takes place, you will be informed of the event but will only need to contact us if you do not wish your child to take part. Consent will be assumed in all other instances.
If we believe a trip/visit may be contentious, we will ask for additional consent at that time.
To further reduce the paperwork involved, we will no longer issue additional SOE3 forms for residential trips.
It is therefore essential that, should there be any changes in your or your child’s information, you inform the College immediately – by phone or preferably email (School Gateway).
